Individually Identifiable Health Information
Haywood County Emergency Medical Services knows that the privacy of
your personal information is one of our highest priorities. We want to be sure
that you know why we need to collect personal information from you. We also want
you to know how we protect your privacy with the measures we take to safeguard
your personal information.
Customer
Information
Haywood County EMS collects and keeps only information that is
necessary for us to provide health care services, requested by you and to bill
for said services. We collect non-public personal information:

Information
We only share your non-public information with other covered
entities or health care providers with a direct treatment relationship for the
purpose of medial treatment, payment, and health care operations. We disclose
non-public information to your health insurance carrier for billing purposes or
to Business Associates under contract with us for the purpose of collections as
permitted by law.
When we provide personal information to a service provider, we
require said providers to agree to safeguard your information and to use the
information only for the intended purpose and to abide by the applicable law.
Your non-public personal information may be disclosed in compliance to a legal
subpoena in conjunction to a legal or investigative matter. Otherwise, your
non-public personal information is only available to you or your Power of
Attorney.
How We
Protect Information
Only employees with a valid business reason have access to your
personal information. These employees have been educated on the importance of
maintaining confidentiality and security of this information. They are required
to abide by our information handling policies and practices. We maintain
security standards to protect confidential information, whether written, spoken,
or electronic for both protection and integrity of your information.

The notice of Privacy Policy is to inform patients on departmental usage and disclosure by Haywood County Emergency Services of the individual patient’s rights and our responsibilities with respect of protected health information used. Haywood County EMS recognizes and respects the privacy of our patients. We are committed to safeguarding your personal information. This privacy policy is for informational purposes. We will update and distribute it as required by law. It is also available from our business office at 828-454-0629 between the hours of 9:00 AM and 4:00 PM Monday through Friday.
