Back to School Forms

If you have not already done so, there are three different forms you need to update for each child you have enrolled at HCES. You should have already received email notices about the Eucharist Permission Form and the Consent for Medical Treatment Form. There’s one more that’s very important – Emergency Contacts. New families will need to complete the entire emergency contact form. Returning families will need to log into their student's profile and review the emergency contact. See below for instructions. 
 
Please log into the school’s website and select your child.  Under your child, find the Emergency Contacts and review the names and contact information for each person who should be called in case of emergency – if you are not available.  These names also include persons who are permitted to pick up or check your child out of school.
 
If you need a refresher – look on the Resources page for the Video Tutorial button, and select the video on Emergency Contacts. You can also update this information during the school year if circumstances require.
