The state of Florida requires all students entering kindergarten and seventh grade to have a Florida Certificate of Immunization (DOH Form 680) documenting all the mandatory vaccines. This form is required to be on file before the start of the school year.

Kindergarten students are required to have: 5 DTP/DTap, 4 Polio, 2 MMR (measles, mumps, and rubella), 3 Hepatitis B, and 2 Varicella (chickenpox) vaccines.

Seventh graders are required to have all of the immunizations listed above, plus 1 Tdap (tetanus, diphtheria, pertussis) booster vaccine.
