Back to School Forms
If you have not yet completed your back to school forms, please see below. It is essential that we have the most up-to-date information on file for all HCES students. Thank you for your help!
New HCES Families
Log in to the HCES website to access these required forms for each child. Begin by clicking on the yellow banner.
- Medical Consent and Information Form: list medications, allergies, and health history for the school nurse and chef
- Blessing/Eucharist Permission Form: 2nd - 8th grade students only
- Emergency Contacts Form: Give permission for each person who can pick up your child from school, complete with their contact information (don't forget to include yourself).
- PK/K Information Form for new PK3 - Kindergarten students.
- Demographic Form
Returning Families
Log in to the HCES website to access these required forms for each child. Begin by clicking on the yellow banner.
- Update Medical Consent and Information Form: list medications, allergies, and health history for the school nurse and chef (don't forget to add those "as needed" over-the-counter meds approved for the nurse to administer).
- Blessing/Eucharist Permission Form: All incoming 2nd grade students, only
*3rd - 8th grade students - if you would like to make a change to an existing Eucharist permission, please email Irene Engelbrecht at iengelbrecht@hces.org - Emergency Contacts: Go to the "Contact Card" tab and update permissions for each person who can pick up your child from school, complete with their contact information (don't forget to include yourself).
