Immunizations, Health, and Medication Forms

Health and immunization forms should be up-to-date and on file for all children. Immunizations must be documented on the Florida Certificate of Immunization. This form is available from your physician or may be downloaded from the Florida Shots website (www.flshots.com) using your child’s Florida Shots ID Number. The ID number is available from your physician’s office.

The HCES Medication Permission Form (attached below) must be completed before prescription or over-the-counter medications may be administered. All medications will be stored in the school clinic and dispensed under the supervision of the school nurse. Students are not allowed to self-administer any medications.

Forms must be on file for your student(s) before the first day of school.

- 4 DTaP
- 3 Polio
- 1 MMR (Measles, Mumps, Rubella)
- 3 Hepatitis B
- Hib--doses will vary; typically 3 or 4 but may have only one
- 1 Varicella (Chickenpox)
Kindergarten
- 5 DTaP
- 4 Polio
- 2 MMR (Measles, Mumps, Rubella) 3 Hepatitis B
- 2 Varicella (Chickenpox)
Seventh Grade
- All the immunizations listed above plus 1 Tdap (Tetanus, Diphtheria, Pertussis) booster vaccine

Mrs. Gabbi Oates is a registered nurse with 30 years of experience and most recently worked at the Department of Health. She has worked at several hospitals during her career including locally at Tallahassee Memorial.

Health and Wellness Recommendations

Frequently wash hands with soap and water for at least 20 seconds or use hand sanitizer with at least 60% alcohol.
Avoid touching eyes, nose, and mouth and cover cough or sneeze with elbow or tissue.
Clean and disinfect frequently touched items.
If you feel sick, please stay home.

Sick Policy
Holy Comforter School encourages a healthful environment for all of our students and employees. Please do not send your children to school if they are sick and notify the school immediately if your child has a communicable disease. Students too ill to function in the classroom are referred to the nurse. She will determine if students should be sent home and make arrangements with the parents. The guidelines below are listed as a reference when determining if students need to stay home from school.
- Strep Throat or Impetigo - Stay home until after receiving antibiotics for 24 hours and/or cleared by a physician to return to school.
- Fever - Students who have a fever (above 100.4°) cannot attend school or any school function (musical, field trip, athletic event, etc.) until fever-free without medication for 24 hours.
- Diarrhea and Vomiting - Students should stay home from school if they had diarrhea or vomited within the last 24-hour period.
- Ear Pain, Chronic Cough, or Nasal Discharge - Please contact physician.
- Lice - Students may return to school after treatment
- Seasonal Flu - Stay home at least five days from onset of symptoms.
