HCES Flu Vaccine Clinic - Thursday, October 20, 2:00-4:30 PM
Flu shots will be available for students, parents, faculty, staff and church members. Children under the age of 18 must be accompanied by a parent or guardian.
The flu vaccine being administered is a preservative-free, quadrivalent flu vaccine which protects against four strains of flu.
The flu vaccine is available at no charge to individuals who have Capital Health Plan (CHP) or Blue Cross Blue Shield (BCBS)—State Employees Group. Individuals with CHP OR BCBS of Florida must bring their insurance card and copy of their driver’s license (valid ID). For individuals without either of these insurance plans, the cost for the vaccine is $28.00. Payment may be made by check, made payable to “Florida Dept. of Health in Leon County”, or cash.

Please fill out a registration form for each person receiving the vaccine and attach a copy of your insurance card. Pre-registration is requested. Please turn forms into the lower or middle school office by Wednesday, October 19.
